What this works out, and what it asks you for
Social Security and Medicare are fixed percentages of gross pay, 6.2% and 1.45% on the employee side, so those are calculated for you and will be right.
Federal and state income tax withholding are not. What comes out of somebody's pay depends on the W-4 they filled in and on the state they work in, and two people earning the same wage in the same week can have different amounts withheld. A generator that guessed would hand you a document with wrong figures on it, which is worse than no document, so those are fields you fill in from the payroll run you already did.
Two edges worth knowing if they apply: Social Security stops being withheld once someone passes the annual wage base, and an extra 0.9% of Medicare starts above $200,000. Neither binds for most hourly staff.
What has to be on a pay stub
There is no single federal rule. The FLSA requires you to keep the records, not to hand anyone a stub, and it is the states that decide whether one is owed and what it must show. Most that require one want the pay period, hours worked, rate, gross, each deduction itemised, and net pay. Several also want your business name and address on it.
Which state you are in changes the answer, and a few require the stub to be printed rather than emailed unless the employee agrees otherwise. Pay stub requirements by state has the rule where you are.
Getting the hours right in the first place
The figure this stub stands or falls on is the hours. Adding up punch cards by hand is where the errors come from, and an underpayment you cannot disprove is the expensive kind.
